Child Processes: Unread stderr Hangs, Dead Readers Cause Crashes

A stdio server that had passed its smoke test 9/9 froze after I added debug logs to stderr at 4 KB per line because the caller opened a pipe but never read it. On the other side, a child process that must keep running after the UI closes fails every time it prints if its stdout is a pipe whose reader has died.
